Credentialing of physicians and other health care providers

All physicians and other health care providers joining the Humana network have to go through a credentialing process. One of our Physician Contracting Representatives will provide, among other documents, a credentialing application during their first office visit. You need to complete the credentialing application and provide all documentation specified in the checklist, through e-mail at prcredentials@humana.com or fax at 1-866-423-6635. The checklists are identified below by area of service. To complete the process, once all the documents have been received, a Physician Contracting Representative will contact your office to schedule an appointment and collect the signed contract and perform a site visit inspection.

Re-credentialing of participating physicians and other health care providers

If you already are a participating provider, you are responsible of submitting credentials that are due annually. Humana re-credentials providers every three (3) years.
Once the documents are completed, a Physician Contracting Representative will contact your office to schedule an appointment and collect the credentialing documents and perform a site visit inspection, thus, completing all required documentation to submit to the evaluation committee.
