How to join the Humana network

Providers in Puerto Rico may request to join our network by completing an online Service Request Form. Also, you may contact our Humana Customer Service line at 1-800-611-1474. Some Humana networks may have additional participation criteria providers must meet in order to be granted participation status. As part of the contracting process, all providers have to pass through the credentialing process.
